Closed Files

Client records are the property of Dr. Swaine and kept in a secure location under his supervision.  They are not kept electronically.  In the event of Dr. Swaine's incapacity or death, a psychologist designated by him will be responsible for the files.  This individual's name can be obtained from The Nova Scotia Board of Examiners in Psychology office.

 

  • If you have not been seen in twelve or more months, your file is considered closed and is stored in a secure location.
     
  • Copies of closed files or closed file material will be transferred to appropriate institutions or individuals only with your explicit written consent.  Original file material will be retained in secure storage.
     
  • Your file will be reviewed by Dr. Swaine or another psychologist designated by him, so that private content will not be inappropriately transferred to others.  You may be contacted to verify your identity and the specific material you wish to release.
     
  • There will be prepaid charges for these transfers, to cover the costs of file review, reproduction and delivery.
     
  • Adult client records are kept for at least ten years following the last client contact.  After that time, the client record will be destroyed.
     
  • If an individual client was less than eighteen years of age at the the time of last contact, the individual client record is retained for ten years following the date of the client's eighteenth birthday.  After that time, the client record will be destroyed.
